The Adult/Adolescent Sensory History Scoring Program© is designed to score the Self-Report Questionnaire and Caregiver Questionnaire. 

• The test administrator enters demographic information as well as item responses into either a fillable PDF, or excel spreadsheet.
• A one-page report form, accounting for missing items, is generated automatically.
• The report provides raw scores for all test subscores as well as score interpretations. With the PDF version the test administrator inputs z or standard scores using the lookup tables provided, the excel spreadsheet automatically provides z or standard scores.
• The single page report form is suitable for storage in client records, and may be printed to give to clients.

• Requirements: Adobe Reader (free) or Microsoft Excel.One license provides unlimited use for up to 30 users.
• Program may be installed on 2 computers or 1 server.This product includes one (1) fillable PDF, one (1) lookup table and one (1) excel spreadsheet, provided as downloadable files.
